1. Identification of the copyrighted work.
2. The exact FanReviews.com URL.
3. Your contact information.
4. A good-faith statement that the use is unauthorized.
5. A statement under penalty of perjury that the notice is accurate and you are authorized to act.
6. Your physical or electronic signature.

Fan Reviews may remove or restrict access while reviewing a complete notice and may forward it to the relevant platform or provider. Knowingly misrepresenting infringement may create liability.
